Alzheimer's Disease Programs Initiative (ADPI) - Developing Dementia-Capable Community Health Worker Programs in the National Aging Network

ACL plans to award cooperative agreements to develop and expand dementia-capable community health worker (CHW) programs within the National Aging Network. Projects will integrate CHWs into community-based aging services to improve dementia screening and referral, deliver dementia-specific evidence-based or evidence-informed interventions, reduce barriers to care, and improve access to supportive services and quality of life for people living with dementia and their caregivers.
